You need to give AmeriPlan 30 days notice in writing that you wish to cancel your AmeriPlan Dental Plan membership. You may cancel your membership in writing by mail to 5700 Democracy Drive, Plano, Texas 75024, fax to (469) 229-4595, or email to stop@stopmembership.com. Please be sure to include your name, address, phone number, membership number, and reason you wish to cancel.
