Dear Administrator and Directors

We are past and present co-workers and victims of the following individuals who are dangers and liabilities to the 1) health, safety, and welfare of the public and our families in our community where we live  2) employees,  clients , co-workers, customers,  and visitors in the work place in which we work and 3) to the youth,  women, and seniors in our healthcare facilities in which you have administration and ownership.  Their status is briefly outlined as follows for your reference and action:

ANNETTE R RICHARDSON
57 years of age living with her husband Thomas A Endicott in Phoenix Arizona
Arizona licensed senior assisted living facility administrator
Pacifica Senior Living and Encore Senior Living recently requested her to leave her employment as administrator at Pacifica Senior Living Paradise Valley Arizona due to her questionable performance and unacceptable conduct in the workplace. She was allowed to resign in exchange for termination.
Presently employed by Brookdale Senior Living at their Brookdale Freedom Plaza Retirement Community in Peoria Arizona as a sales agent in the call center.

THOMAS A ENDICOTT
60 years of age living with his wife Annette R Richardson in Phoenix Arizona
Charged with 72 counts of healthcare fraud and given two criminal felonies for healthcare fraud by State of Michigan
Charged with 19 drug counts and given a criminal felony for unlawful and illegal distribution of controlled substances by the State of Michigan
Charged for criminal violation of student safety zones by the State of Michigan
Convicted of hostile work environment and criminal sexual harassment of women in the workplace by the State of Michigan
Michigan Board of Dentistry and Michigan Department of Community Health suspended his dental license and has refused to allow him to practice in the State of Michigan
Placed on $1.4 million bond due to flight risk by the State of Michigan
Sentenced to two years of imprisonment by the State of Michigan
Multiple malpractice and disciplinary actions in both the State of Michigan and the State of Illinois
Arizona Board of Dentistry refused to grant him permission to practice dentistry in Arizona
Recent Order of Protection entered by Judicial Court Branch of Arizona against him for dangerous harassment of wife and the serious beating injury of his son in law

THOMAS S ENDICOTT
74 years of age
Charged with six felony counts of sexual abuse and transporting minors across state lines with intent to sexually molest and engage in sexual activity with them
Sentenced to 8 years of imprisonment by the US District Court with the assistance of the US Attorneys Office and US Federal Bureau of Investigation



Due to the unstable and volatile characters of these individuals we believe that there are upcoming events and activities that will cause grave danger and liability for additional employees,  co-workers, clients,  customers,  patients,  and visitors in our work place, and to the youth, women, and seniors in our healthcare facility.

In light of the above we pray that each of you as administrators and directors will immediately take the proper preventative and proactive action in your company, agency, and department to limit, restrict, and cease the employment or licensure of these individuals so they cannot continue to administer or practice more abuse, danger, and harm in our community and our workplace.

Thank you for your support and understanding of our fear, cause, notice and attempt to prevent further hostile abuse and harassment to children and women in our community and our workplace.

We and other co workers and victims continue at this time to remain anonymous for fear of these individuals and worry for our safety and our family's safety at all times in and outside our workplace and at our homes.
