I signed a 1 yr gym contract, paid for monthly out of my ckg acct. ABC company collected this money for Arizona Swim & Fitness. After the year I continued to pay/use the gym on a month to month basis (per the contract) for several months after. In order to discontinue this month to month deal (per contract) I was required put my request to cancel in writing to the gym 30 days in advance. I did so (per contract).

3 months later got notice from ABC stating my ckg acct was not longer a valid acct and to please submit new acct info immediately to avoid cancellation of my Gym privileges. I called ABC and explained that I had cancelled my membership 3 months ago (per contract). (note: my chk card was stolen and I closed that acct at the bank). ABC informed me that the contract they had required me to send a notorized letter to "them", not AZ S&F, in order to discontinue.

I reviewed my contract with a magnifying glass and found this statement nowhere. I went back to my bank statements and realized that they continued to take the monthly fee from my acct. Yes, I should have noticed, but..this isn't the point. I asked this company to send me a copy of the contract they say I signed and 4 months and many threats later, I got a copy.

This copy has additions, handwritten, in different hand writing then the originators. My copy is the original Yellow carbon copy and it does NOT have this addition. Of course the addition states the clause about the notorized letter requirement. Just got a collection notice for $299. Time to seek an attorney. Keep copies of these contracts.even when you feel you have satisfied them.
