In August 2010 I was scheduled to go in for a narrative (basically a prognosis of to what extent your injuries will affect you, etc). I was told to go to their Juniper Island office and even asked twice to confirm (I was leery because up until that point I had been going to their main office).

When I arrived, they were baffled as to why I was there and made me redo all of my "new patient" paperwork again because they couldn't find it in their system. Needless to say, my appointment started an hour later than what was scheduled.

Upon completion, the chiropractor told me that he was a little behind on patient paperwork and it would take about a week for him to write it up. I was perfectly okay with that.

A month goes by and I hear nothing (they were going to mail me a copy). I call only to find out that absolutely nothing has been done with it. They blamed it on a nurse in the office saying that she "dropped the ball on a lot of things" and no longer worked for the company because of it.
