On April 6, 2015, the date of my mother's death, I requested a Claimant's Statement (death benefit claim form) from Bankers Fidelity Life Insurance Company, which was not received. Subsequently, I made numerous requests for the form with negative results. A claims representative advised me to seek assistance from the funeral home instead. (Bankers didn't receive their premiums from the funeral home, why should I obtain a claim form from the funeral home?) I suggested to the claims representative that I use the downloadable form provided by Bankers' website. The claims representative advised that such form would not be excepted.

The form must be on green paper and bear the company name and address. (The downloadable form would be on white paper and bear the company name and address.) I did in fact use the form, which Leevy's Funeral Home submitted on my behalf on or about April 21st. Finally, on April 30, 2015, Bankers Fidelity Life Insurance Company emailed a beneficiary's death benefit claim form to me because I was unable to receive it by first class mail. Why wasn't the claim form emailed to me sooner? In view of the foregoing, I would not recommend Bankers Fidelity Insurance Company, unless you want your beneficiaries to be tortured by the unscrupulous practices of Bankers Fidelity Life Insurance Company.
