1. I rented a heart monitor at Good Sam Hospital, Downers Grove, IL during Sept., 2019. The supplier used by the hospital was Cardiomedix.
2. During the month I was notified that an apartment I wanted at a Senior Citizen Community in Iowa was available.
3. On Sept. 30, 2019 my wife and I moved to Iowa with the help of two of my sons and one grandson. During the move I took off the monitor. A few days later I realized it has disappeared.

5. On Nov. 3, 2019 I decided the monitor had been accidentally disposed of. I mailed a check for $800 to the company.
6. On July 10. 2020 my wife found the monitor packed in the side pocket of a suitcase.
7. I notified Cardiomedix. They provided a envelope and box to return the item.
