I have received an unsolicited offer for a questionable "pre-approved" credit card. I DO NOT WANT THIS CARD, but there is no way to formally turn it down, or to decline the offer, based on the information offered.

I fear that this card will be activated without my permission. Is there anyway I can contact this company and formally decline this card?
