Mailed in rebate form, original receipt, & bar code from LouAna 3 gallon Peanut Oil required to receive $5. for purchasing CMS Rebate Center, Lou Ana $5. rebate, Program LouAna 1201, P. O. Box 426008, Del Rio TX 78842-6008 on 2-24-2013. Was supposed to receive by 3-24-2013. To date, I have not received the rebate.
