Several issues with this company.  First we requested a duplicate life insurance policy because on was lost in the process of moving.  The company refuse to send one.

A life insurance policy was written on my mother. At the time she listed both my brother and myself as primary beneficiarys.  My brother subsequently passed away and documentation was submitted to change the beneficiary to me as the single primary beneficiary.  The company was American Pioneer.  Constitution Life acquired American Pioneer.  A letter was sent to Constitution Life for a duplicate policy again none was receive.  Constitution Life saw fit to only send a Certificate of Insurance.  The certificate name me as sole beneficiary.

Upon my mothers death a claim was filed for the death benefit to pay for funeral expenses.  While Constitution Life was fairly prompt with the claim they only sent 50% of the death benefit and said the needed the death certificate for my brother, which had passed away several years earlier.

I explained that I had an endorsement in my hand from American Pioneer that the beneficiary had been changed and also a Certificate of Insurance from Constitution Life listing me as the only beneficiary.

They claimed they could find no record and wanted me to fax a copy of the documents.  They also said that to receive the balance of the death benefit it had to go back through the review process.  This is a travesty especially with the claimant is going through the greiving process and attempting to use the funds to pay for a funneral.  Proper steps had already been taken to insure the claim could be properly processed and paid according to the policy.

Maintaining proper records management is a fudiciary responsibility of an insurance company.
