We discovered that Premium Long Distance (www.PremiumLongDistance.com) had been billing us for months for long distance service we were not using and never asked for.  When contacted, they said that my wife had applied for it in return for $1,000 in grocery coupons on www.consumerincentiverewards.com.  They gave me as proof the last four digits of my wife's SSN (which were wrong) and my wife's birth date (which was wrong).

This was clearly fraud on behalf of www.ConsumerIncentiveRewards.com, but www.PremiumLongDistance.com was also complicit. Having received our phone number and the fraudulent application for service, Premium could easily have called that number and verified that we asked for the service. They don't because they know that Consumer Incentive Rewards is dirty but delivers them revenue.
