Complaint Review: design surfaces tile and stone

Jeffrey Court tile product purchased through the distributor, Design Surfaces. After installation failure of product occurred. Following the initial tile failure it was noted that excess manufacturer netting adhesive was "globbed" sporadically on the tile sheets.

These "globs" did not allow the setting mortar to adhere the affected tiles to the floor resulting in the tiles releasing and popping out. Once this failure was repaired, at my expense, another failure occurred and more tiles popped loose.

I tried to initiate a warranty claim only to be ignored when requesting the proper forms. The tile is obviously defective and the company is dropping the ball. All I am requesting is reimbursement for the initial product purchase because I do not know how many more tiles may fail at a later date.
