we have ordered 2 ezycash.net Cirrus cards.
they use the following contact details:

Contact Information
Address:
101 Convention Center Drive, Suite 700
Las Vegas, NV 89109
Mailing Address:
P.O Box 27740
Las Vegas NV 89126
Email Address:
info@ezycash.net
No phone number. The card is issued by Omni Bank.

To load the card, this is what we needed to do: (quote from their webpage)
"Instructions:
Utilizing your banks TT service or online overseas payments service enter in the following information:
Beneficiary Bank:
OMNI Bank
2900 Ridgelake Drive
Metarie, Louisana 70011
Beneficiary:
FSV Payment Systems Ltd.
ABA Routing #: 065003453
Account #: 3500012731
Place your 16 digit card number in the message section with your last name.
Once you have sent the funds, please forward via fax a copy of the Overseas Telegraphic Transfer receipt to +61 7 3852 2501."
Why does an US compay have a fax number in Australia ? This looks kinda weird. The thing is that on the instructions that came with the card, it sais that to load the card, a TT transfer should be sent to Stillwater National bank and Trust, Oklahoma. Anyway, after we loaded the card (aprox. 4000USD), the card became inactive, and they do not answer emails nor fax. I think someone should investigate this.
The card is issued by Omni Bank, used by FSV Payment Systems, and i do not know where ezycash.net comes in, not what is the relation between them. I have the card, and all the papers that came with it.
Anyway i belive that its not safe using their services , is rather kinda unsafe. After registration, before sending funds, they said that there is no problem, and that we should use TT tranfer to Omni Bank for loading the card, now the things have changed.

I live very near this address, and have been in this building before.

If it would be helpful, I would be happy to "pop in" and see what's up. I know how to gently get whatever info is available. Also, I believe I did business with the property management company for 101 convention center, in 2000.

Dennis, I know the British have a history of shouldering through any dispointment, but I admire your stoicism. Email me at [DELETED], and I will send back whatever info I can glean. And thank's for playing The Star Spangled Banner at the Changing of the Guard on 9-12-01.

Jim
