A San Francisco hospital needed to see my Mammogram x-rays when I (& the mammogram center) discovered a drainage tube had been left inside me during surgery at this hospital.

I then had my mammogram x-rays sent from the radiology center to the S.F. hospital (approx. 12 miles away). When I decided to have the surgery redone & drainage tube removed by a physician close by me and not to return to the S.F. Hospital for the surgery (since the physician there had left the drainage tube in), I called and asked the hospital to return my x-rays directly to me so that I could give them to my new doctor.

The hospital contacted Federal Express and arranged to send them to me "overnight delivery". They were lost by Federal Express, which Fed Express acknowledged to the hospital's administrative person many times. The hospital's administrative person then filed a claim form with Fed Ex after I told her I had to have new ones done for my doctor since the original ones were lost, which I did. The total amount of the new mammograms was $268.00
The fed express tracking number was 824519958608; the control claim number is 0513024694.

The hospital, on the claim form they submitted, indicated the check should be issued to me and put my name and address in the "Claimant's Name & address" field (I have a copy).

I have called and emailed them on several occasions; spoke to "Amanda" at least twice, and another time to "Suzanne". They acknowledge they have the claim form, but no one ever gets back to me and subsequently, I have had to pay twice (total of $268.00 each time) for mammograms. They refuse to send the money to reimburse me.

Incidentally, the x-rays finally showed up - 31 days later! They had been opened and tampered with; all my personal information was on these ex-rays to add insult to injury! By that time, needless to say, I had already had new x-rays done for my doctor here as he required them asap.
