I sent you an e-mail over a month ago to fill this w9 out and fax it back. You never replied and never faxed it to me.

I told Kim that we will not pay you until we receive this. Once we receive your w9 we will be able to issue payment.

You need to fax it to 216-420-9701.

This will be the last e-mail that you receive from Integrity. If you do not fill out, sign, and fax back the w9 you will not receive payment. Quit crying about your money, if you would have responded to my e-mail or just simply faxed

the w9 back you would have already received your check.
