Notified Vicky at Ladies workOut Express in November that I was going to have total knee replacement-she said to call 800-658-7727.I did and was told they would send me a Verification of Disability for. Weeks later received form and had my orthopaedic surgeon fill it out and fax to National Fitness and send the form back to me. When I received the form back, I called and was told they did not receive the form. I faxed the form to them. I called to check and was told that it takes a long time to be processed. I had a bad feeling about this situation-already automatically deducted 24.95 for November in my checking account. I stopped payment for December 23 payment- Thinking everything was cleared up by then.

On Friday January at around 4o'clock pm, I received a phone call from Louie at National saying that I was pass due. I explained the situation and was told they did receive the fax but It was not filled out correctly!I asked to speak to a manager and was told I had to fax my request and a manager would call me back??? I asked for a supervisor and was told Louie was a supervisor! I was told that the doctor didn't who signed the form and filled it out-didn't say how many years I would be disabled???As ridiculous as this is, I was told to have my doctor fill in the number of years and/or check the box that says a perminent condition.

On Monday Jan.12th I had an apt. with my doctor and he changed the form as instucted and initialed it as instructed. His nurse faxed it. On Tues. Jan 13th I receive a letter telling me I owe 39.95 as the amt for return of automatic payment and unless I dispute this within 30 days a judgement would be sent to me. I am getting ready to call National as soon as I get rid of this headache.
