Dear Customer Service

On January 29th, 2013, I found out I was slammed into a long distance
provider I never approved. I found that I was slammed while on vacation in
Wyoming.   I asked that the confirmation in my supposed recorded voice be emailed to me,
as I know I did not give my approval.  There is no way I would have approved
changing my long distance carrier as I was happy with my previous service which
was recently changed in November of 2011 with AT&T. 

I was asked my date of birth on January 29th, 2013, by your staff, which
could have been dubbed into any conversation and easily obtainable in online
search engines.  I was also told by staff at LDCB that anyone could say they have
authority to change the service and it can be changed.  I am dumb-founded at
this statement.  This is the true epitome of being slammed. 

To date, I have not received an email enclosing my permission to change our long
distance service as there cannot be one anyway!  I am the only one in the office, and the only
one who makes any changes to our phone service.  I did not
authorize this switch, and demand to have the fees returned to my account. 

I am reporting this to FCC as well as BBB. 
Please be aware I will post my outrage in every avenue I see fit should
this not be rectified asap.
