I received a confidential Privleged Privacey Right Contract from 2005 dated being sent on 11-18-2005/ 11-19-2005, with a confimed status of the 18th of November 2005.

Another form came with a notification of the winnings titled: FORM W-915

FILE REFERENCE BOX # N1Z1A-2005 213-953

ELIGIBILITY BOX: Pre-approved # of 5033245836 under the bar code in the 2.

Initials L.H.R. was on the stamped seal and also possible with the handwritted signature on the W-915 form.

As $20 fee was asked to be sent back with debit information to take from my account for a "one time fee." Then the deposit of my winnings of $3,700,000 would be deposited from my winnings.
