Dear Sir,

I got put out of work due to an injury on the job. I have been out of work for a year now. I'm a disabled male and do not'know if I will be able to return to work.
I've paid $12 for application fee, and $65.95 for additional charges. Checks written on 5/3/03 and 5/5/03
Still waiting to receive materials to do home business.
