Dear Scott, this prize money has been verified and in a report for ID holder and there is a number, my name in full, and the amount of 1,250,000.00 that this is on record with their company, blah, blah, blah, please sign & send attached form to attendant reporting office make sure to include the attendant reporting office fee the 19.99, when recieved in our main office delivery will be prompt.

I have not sent anything to this company but I wanted to warn people about this.

There are two different towns and Zip codes.
(actual example) envelope has this address on it.

The actual letter has this return address
P.O. Box 9101
Bethpage, NY 11714

I have also recieved phone calls on the same matter for prizes of such fortune or more.

Understand this if you have won a real prize the company will not charge you a fee they will send or deliver it straight to your door, they will ask you for a verification of ID before they hand you the check.
