I was hurt working for Pets Plus on July 13th,2006. While doing my work, a 50 lb. safe on an unstable shelf, FELL onto my head, giving me a concussion. When I was taken to the emergency room, I was out of it and let the hospital have my insurance information. We were unable to contact the owners of the company when this happened.

When I spoke to them the next day, they asked me to leave it under my insurance and to forward them the deductable. I really shouldn't be shocked that the measly $100 bill is still unpaid (the entire bill was $11,371). I was terminated under false accusations in 11/06 after 4 years of employment, searing my bond and loyalty to the company, and I suppose leaving them under the impression that this bill is no longer their responsibility.

I now know how stupid I was for being so loyal and trusting that I would be taken care of by a company I worked so hard for. Because of the slip-up at the hosital, it was not filed as a Workman's Compensation claim by the hospital, and never by the company.
This is not the first time this has happened, and shame on me for not seeing the red flags the first time around.
I broke my hand working for them in 2003. I was out of work for 1 month. Pets Plus never filed this as a Workman's Compensation claim, eventhough the hospital had. Because of this all bills for treatment (including a $3000 MRI bill), were reported to collections under my name in 03/06.(Pets Plus fianlly paid it off then)
While I missed the month of work (July of 2003), I was never paid for it until Feb of 2004 after much begging and pleading.

While my former bills are finally taken care of, I still find myself practically begging to have this final bill for my concussion taken care of. Simple enough, I could pay the $100, but it is not the point. It is about to be reported to collections within the next few days.
Who's responsibility is this?
