 hello jan liss is a maneger at the public welfare office and in april of 2011 she was aware that a jessica duck was on  a case record as head of house hold and really this lady should not have beenon that case as head of house hold .this case was a disabeled person who was almost 50 years old and was in danger for atleast 3 months because of what jessica duck has done after a complaint was filed against jessica ducks father in a upmc clinic back in jan of 2011. we believe that jessica decided to submit a claim form as head of household of the consumer. and make it very hard for the consumer to get care on her own and it actualy went through and was submitted. without the consumer knowing that jessica duck was her head of household. and jan liss should have reported jessica duck because jessica duck works within the electronic medical records department and she can do this again.

ther is a police report   and miss jan liss knew it but still she just removed jessica from the consumers records but did not report her  and my qestion is why is it that the medical staff and the people who work in the medical system are not reported and can do this and what happens when a consumer passes away because a consumer was turned away from a doctor  becuse of not knowing the head of her household. this is what the consumer in this story went through for 3 months whom is  epalepsy asthma touretts syndrome

I think know what the OP is saying.  A woman named Jessica Duck angry at the treatment of her father fraudulently claimed to to be the head of a disabled person's household.  This was done to deny that person medical treatment.  She is also angry that once this was reported this to the Pennsylvania Welfare Department  nothing was done to Ms. Duck.    The OP mentioned a police report she needs to follow this up with them and escalate as necessary.

I tried to make sense of that but really couldn't.  In addition to spelling, a little use of punctuation and sentence structure would be very helpful. 

Do people even read what they are submitting before they submit it?

 hello jann liss at the welfare department at 914 penn ave #500 new that a clinic medical record tech has comitted fraud  by submitting a form to the welfare office claiming to be another consumer as head of household and mrs liss never reported the lady whom was jessica duck, jessica duck forged a claim form signed a consumers name and welfare excepted it in april of 2011
