Sent $20.00 mail- in rebate to QPS Que Products Rebate- Dept. 33697POB 134 Niagara Falls, N.Y. 14302, 7/02.
In addition I enclosed J2R Music World receipt, original UPC code and serial # sticker.

Although I have written to them 3 times, and enclosed copies I have never recveived the rebate
