Complaint Review: Reservation Rewards WebLoyalty.com

Last November I shopped at printpal.com for reman toner cartridges (a good deal) then got a screen offering $10 off my next purchase.  Filled it out but saw no mention of recurring charges.  Just noticed on my bill charges from 2 companies: reservation rewards and travel values plus for $12 each.  Called and cancelled complained and was refunded $72 from each company and sent a form which the phone flunky said I would need to complete to get my remaining refund.  It was titled  UNAUTHORIZED CHARGE AFFIDAVIT.  I did and will not complete it.  Instead I went to the CT Secretary of State and found webloyalty.com in their corporate business listings. The address is http://www.concord-sots.ct.gov/CONCORD/online?sn=InquiryServlet&eid=99
