In Nov 2009, I received info request from UHC to complete to continue COBRA insurance into the year 2010.  I completed the form and mailed it back to them, omitting the section that said - Complete if you want to make changes in coverage - I assumed that this was a change from the current coverage that we had paid since February 2009.  Upon receiving the new years statement for 1/1/2010, the coverage for Dental care had been left off. On 12/21/2010, the Monday after the invoice was received on the prior Friday, and ask that this coverage be added back.  I spoke with a young woman and she put me on hold while she spoke with her supervisior and got approval to continue this coverage.  When making the payment for Jan 2010, I included the amount for the dental coverage as instructed.  Then I spoke to Larry on 1/20/2010 and  followed up on  my request and said that I would receive confirmation in 3 to 5 days via email.  Well, since I never received any confirmation, on 1/28/2010, I telephoned UHC again and spoke with Larry again.  His explanation was, the coverage is denied.  He said that there was no record of my calling in December 2009 and that he did not speak with me on 1/20/2010.  I have phone records that show that I did indeed telephone them and on the first and second telephone calls were lengthly. Also, the coverages have been changed to cover less and I was never notified of anyt changes.  I had a Mamogram on 1/4/2010 and I had to pay a deductible. A deductible has never been charged before.  I was never notified of this change.  I want the public to know that this company has become very unreliable and I am very dissatified with the rudness of the customer services.  Thank you,
