USA Credit Benefits ripoff, refusal to refund, suggest ID theft by someone I know Uniontown Pennsylvania

When I discovered the automatic withdraws from my checking account, I called the 800 # and they suggested someone I know was stealing my info and had applied for this using my info. They sent me a 4 pg. document checklist which unless I file out and return, they will not refund my money. Most of the check list deals with identy theft, which is ironic since they are the real theives. I have reported them to the Pennsylvania BBB, and on Monday, since they close at 5 and on the weekends, I will be faxing the VP, Kimberly Coddington a letter stating that if my money isn't re-deposited in my account w/in 24 hours I will report them to the fraud squad at the police department. I never signed up for this and have no idea how they got my info.
