Wireless Advocates Stuart Lee Rebates No Costco rebate card received for 2nd Verizon GS6 Lynnwood Washington

I submitted all the forms to receive rebates for two Samsung GS6 phones and 1 iPhone 6 Plus back in early September 2015 (phones purchased from Costco #17 on 8/21/15). As per rebate instructions, I included the copies of the paperwork provided by Costco along with a copy of my credit card receipt. It was very clear that I had purchased 3 phones. In December 2015, I received the rebate check for the iPhone and just one cash card for one phone. As per instructions, I contacted Stuart Lee Rebate Center via their website to file a complaint about not receving a second Costco cash card. Rules are that there are up to 5 rebates per customer. The only response I received was to confirm that I'd received the rebate cash card for the one phone. I've received no further communication. I just filed another online complaint on the Stuart Lee Rebate Center website. I am waiting for a response.
