This company billed me for something i did not use! i called and bitched and the guy hung up on me! The second lady took my card number and said she was going to cancel the charges, but she billed me again. wish i new the address to this place.

I know this isn't much, but it is a beginning....
On your report, you have a 661-252____ phone number listed for the Company Name.
If this number is correct, it shows it is in Solamint California and is active with the Pacific Bell Phone Company.
This, at least, gives you somewhere to start.
