There are two parts to the form, part one you must sign your name and date in blue or black ink.
part two you must send a fee of $21.95 for processing fee (19.95)and a postal fee of ($2.00).
This company takes three forms of payments check and money order made out to S.I.R.S OR YOU CAN SEND THEM CASH! (IN THE MAIL)
