driver, yes you can request a DAC report from USIS at their website www.usis.com then click on the link at the bottom "Consumers" and about half way down on this page is a link to the pdf version of the DAC application. you are allowed one free report ever 12 months. just check the box on the form "no charge" if you have not yet requested one. If you have in the recent year it is 15.00 per the form. you can also order it through www.DOTjobhistory.com after signup and payment for them to get it. (this website has no affiliation with USIS.) I must let you know though the report takes 15 days to get and through this website above will be in short form and not the full length one you could get from USIS. hope that helps.
