It seems you gave up too soon with your credit card company. Let the credit card company know that dish network admitted to finding the request and still refused to help you. Your results may be partially or perhaps greatly be dependent upon the credit card company representative you are dealing with. These are people who, with the right words, can fight for your cause. Each time you call the credit card company disputes department, you usually get a different representative. Keep calling until you find someone empathetic or sympathetic enough to your cause and get their direct phone number and extension. Explain to the representative exactly what happened in detail and relate that if 'he' or 'she' helps you, you will write a wonderful letter about how great they were to their superiors. Don't just spout this out, acquire some familiarity first and some sign of empathy and then explain how you will help them if this issue is resolved in your favor. Call back Dish Network and tell them that you will be notifying the Department of Consumer Affairs of your state. If all else fails, you can then direct them to this web site and if they want you to indicate that there was a misunderstanding and the matter was resolved, they should refund your money. Also note that if they don't help you (after they refuse) you will post their NAMES on the web site (get their names first). Look through the complaints on line here, see if anything you read is similar to your situation and relate the info. you mentioned, as well as any other fraudulent promises, etc.. made to your credit card company. If you paid by check, you can still try through the Bank, but it's more difficult this way. Ensure you copy and paste reports similar to yours and fax them over to your credit card company as well.
