The registration site provides the information of to the payment which was mailed As An American Express Reward Card on 8/23/2011 .(www.4myrebate.com- Track a Rebate)
We can provide you additional information of a private nature to replace that card if you call Customer Service at 1-800-286-9146 and refer to your tracking number.
The address it was mailed to is what was provided on the rebate form you sent in. Please call the Customer Service number to verify your address so a replacement card can be sent.
