Item # 1... The company "Tia" refers to as "FIRST CLASS HEALTH SOLUTIONS" use to occupy the same office area as we do prior to our company moving in. We continued to receive this other company's mail for about a year after we moved in, thus we assume they might have gone out of business. We don't know who "FIRST CLASS HEALTH SOLUTIONS" is, what type of business they are (or were), and are not (nor ever have been) affiliated with them in ANY way. Item # 2... The illustrated step-by-step instructions that "Tia" received with her kit clearly states to sign and include the Client Details/Authorization Form when returning a specimen to our facility. Additionally, this form is printed on the reverse side of the illustrated step-by-step instructions that "Tia" used to collect and mail her specimen to our facility. Item # 3... A standard policy in the laboratory screening industry is to destroy a specimen and not have it screened when it (a specimen) is received without the proper paper work. Instead, "Tia" was given an opportunity to send in the required paperwork without having to collect a new specimen. Item # 4... Based on the so few number of clients who have ever neglected to return the required signed Client Details/Authorization Form with their specimen, we believe we know who "Tia" is. Item # 5... Once the paperwork was received, the specimen was "officially" logged in and accepted. If "Tia" is who we believe she is, then she was provided her results and all support materials within published time frames based on the date the required paperwork was finally received by our company. In closing, a company as large as ours will have some consumer complaints - whether they be valid or invalid in nature. When there is a complaint we strive very hard to resolve the issue to meet everyone's satisfaction in any issues found to be valid.
