This ordeal began September 12, 2011 and on Friday, October 21, 2011, it ended, thanks to the Better Business Bureau of Arizona. They assisted me in contacting Team Xtreme Sales LLC. After sending letters back and forth, I finally received a refund check for the full amount of my purchase.
Needless to say, I won't be purchasing anything from door-to-door salespeople in the future. A "No Soliciting" sign is now on my front door. One dishonest person ruined it for the rest of them.

Recent development....
Since the email shown on the website is useless and my emails were returned to me undeliverable, I decided to change the last part of the email address to make it match the website address. That worked! Finally, someone named Andrew Martinick responded. He informed me that they do not accept cancellation requests via email. He went on to explain that they will accept a copy of the receipt (the back must be completed), postmarked no later than 3 days after the sale. (By the way, the website and the receipt do not mention postmarked notices. The wording leads the customer to believe they only have 3 days to mail in their cancellation request.)
I wrote back and told him that I mailed a cancellation request, the day after the sale, via Certified Mail with Return Receipt. According to the tracking, a notice was left on September 16, 2011.
Yesterday, on the local news, an elderly couple said they were scammed by a teenage boy selling books. He pretended to be the nephew of their neighbor. Luckily, they were able to stop the check.

Team Xtreme Sales LLC did not have a website for 2 days...until now. When I checked on September 12th, there was no website. Also, the email I sent to [email protected] (I found this email address on the cached version stored on Google) was returned to me with a FAILURE NOTICE.
Their new website has a Contact Form and also lists the same email address I sent a message to on Monday, September 12, 2011. I submitted a cancellation request using the Contact Form and resent my email. Hopefully, this time, they will respond.
There's also a website LOGIN feature that allows you to type in the remaining digits of the receipt number. It starts with D011. However, that did not work. An error message popped up --
"Microsoft OLE DB Provider for ODBC Drivers error '80004005'[Microsoft][ODBC Driver Manager] Data source name not found and no default driver specified/receipt.asp, line 44"
I'm waiting to see what happens next. Stay tuned...
