There are a few things that are common to virtually all items that are sold via infomercial. The first is that the pitch will always include "But wait, there's more!". Second is that shipping and handling will be a large percentage of the price of the item.
The whole point of infomercials is to separate the gullible from their cash. They sell crap that you would not consider buying if it was sold on a department store shelf. As such, they know that a lot of their customers are not going to be happy when the product doesn't turn out to be the miracle that it appeared to be on TV. Therefore, in order to guarantee their profit margin, they include it in the shipping and handling charge because those are not commonly included on any money back guarantee.
While occasionally a useful product is sold via infomercial, 90% of the time it's a ruse to separate the gullible from their hard earned cash. Paying $15 shipping and handling for a $20 item is a dead giveaway that you are being fleeced.
