Their "21 tax solutions" they even say themselves comes from the IRS' list of programs to handle tax debt. They make it sound like this is a good thing, like you've got plenty of options and "we'll examine all of them". The downside of this is that they never actually have to tell you what they're doing on your case in your initial calls. They're basically saying "I'll sign you up for our 21 possibilities program, you give me your credit card info, and I promise we'll do SOMETHING." That's basically lining a client up for disappointment, as you have no idea what they're going to do and will surely be disappointed at whatever they come up with afterward. Since the answer you get from the IRS often seems to depend on which rep you talk to at the time, the results the reviewer above had are not surprising. Effectur makes it sound like they can lasso the IRS and force a "favorable" outcome, but really it seems like they get bullied into whatever the IRS says by phone, and you have an equal chance of calling yourself and getting lucky with a more generous IRS rep.
