The Arizona Attorney General's office is currently accepting claims for refunds on the CP Direct Case. You should contact their consumer complaint and information center and request a claim form. Claim forms were recently mailed out to consumers on record. Apparently, you must not have received one. I don't remember the telephone number offhand, but their website is AGAZ.GOV. Or is it AZAG.GOV? It's one or the other.
