The content of the letter as follows:
"Official Notice of Prize Award - Congratulations! You have won a sweepstakes prize, and must sign and mail the form below no later than the posted deadline for us to expedite your valid claim to a CASH AWARD now ready for distribution.
I report, it has been confirmed on the above date that the file of _____ was annotated as: WINNER OF SWEEPSTAKES CASH AWARD. Upon receipt of your AWARD VOUCHER below, you will receive a bank check and affix your legal signature where indicated. You must be 18 or older to claim legal title to this bank check in your name.
No payment is required to get the CASH AWARD you have already won, but you must sign and complete the form below immediately, and return it to us promptly. Enclose the Award Voucher in your own envelope and mail to Payment Resources at address on reverse.
