Following is a copy of the email I sent to ACUVUE:

Based upon the rebate offer on your web site, I purchased a one year supply of your contact lenses. On Sept 4, 2008 I mailed the rebate information to you. On Oct. 6, 2008 I received an unsigned form letter from you stating We are unable to process you request because: Rebates for COSTCO purchases are only available through the COSTCO rebate fulfillment process. Your web site makes no reference to exceptions or exclusions for the rebate. I expect you to honor the rebate as advertised on your web site.

I look forward to your expeditious reply.

Additional information: I paid over $200 for these contacts and will return them to Costco for a refund if I don not receive a refund. I will also dispute the charges with American Express...
