I received a letter from collections stating that I owe $5696.58, but I have never used this bank, nor have I ever lived at the address they provided that was used to obtain this card. This company allows people to use anyone's identity to ruin a person's credit. I do not owe this and they said to file a police report. I asked for more information, since you cannot file a report on air, I need something to link thi to, but they won't give me anything. If I could get a copy of the signature page, I could prove I've never signed anything to obtain this card. I have only had one credit card in my life and that was in 2011 and they say this is from 1997. This company knows this is a false claim.

I sent both a fax and a letter stating that I do not agree with there claim, and that I am not the resposable party, I dispute there claim. Is there anything else that I have to do, beside file the police report? I am a little worried that the police reporty will not help  me, since the claim is from where I used to live (TN) and I now live in CA. I hope that I can even file a police report in CA.

The process is much easier than that -do some research right away with the reputable credit sites to find out what you have to do when faced with ID theft. Because you have received your first dunning letter from a collections agency you want to respond right away disputing the claim. There is a time limit of 30 days for you to respond so don't let too much time pass.

Yes, file a police report. They don't ask for a copy of a signed agreement. Then you dispute with the CRA's if it is on your credit report.
