Ordered an item from seller Better Choice Online which is an Amazon seller. The item ordered was a Sapphire Radeon R9 290 Tri-x graphics card. The item was listed for free shipping and the estimate delivery date was March 20th - 24th. Item shipped same day and arrived within 2 days, which was unexpected and I was not home all weekend.

After logging on to Amazon to track my order I then noticed the item said "Delivered". I contacted the seller and I was told to wait until the next week to see if it showed up. I waitied and contacted the seller again....

"Thank you so much for waiting to see if it would turn up.  I’ll go ahead and get a refund set up for you shortly.  When time allows, could you fill out a PS1000 form (attached) so I can file a claim with my insurance"?

At this point I was sent a blank USPS insurance claim form and was told to fill it out, sign my signature and return. I filled out the form and at the bottom I read

"I certify that all information furnished on this form is accurate, truthful, and complete, and that I understand that anyone who furnishes false or misleading information on this form or who omits information requested on this form may be subject to

criminal and/or civil penalties, including fines and imprisonment".

This is a problem because I cannot verify the information on a blank document. Again I contact seller and explain that I cannot legally, honestly and truthfully place my signature on this form without the companies information filled out first. This was apparently a problem for the company and insisted that I fill out the form.

 

"Like I mentioned before, we need you to fill out the PS1000 form.  Once you

fill out the form, then our insurance will pay us and we will issue you a

refund.  If you refuse to fill out the form, they will think that you are

trying to hide something.  Just spend the 30 seconds to fill out and sign

the form and we will get this resolved".

 

But wait... Didn't I already get told to fill it out WHEN TIME ALLOWS?? And a refund would be put in SHORTLY?? This threw up red flags because the first rule of business is you do not accuse or blame the customer. First of all who tracks a package for 11 days? The date for delivery would have ment the item would have arrived Friday - Monday. I order lots of things online and have never encountered a problem with products not arriving or being stolen at my home.

 

My next step was to contact Amazon and file a claim through them. A claim was filed and I am currently awaiting an answer. Meantime I called my postmaster and asked if I should indeed fill out the form and place my signature on it without verifying the other information. I was informed NOT to fill out the papers because that is done by the company not me. I was also told to NEVER fill out and sign a blank form.

 

Contacting the seller again and...

 

"I have just got off the phone with the local postmaster. I explained the situation at hand and I was told to NOT sign any blank forms. Also the postmaster informed me that I am NOT the person filing the claim to the post office I do NOT place my signature on the form it is for you to fill out. If you would like a copy of the recording I will provide you with that. If you would like to contact her yourself her name is......"

 

You will never guess what happens next? I was sent a outdated ps1000 form from 2005 with the information INCORRECTLY filled out. The address listed for the company shows up on google maps as a UPS store next to a KFC and a Wallgreens. The item listed on the form is a GeForce 290 video card which doesn't even exist.

 

Amazon told me the company is under investigation and to no longer respond to their emails.
