After posting this report, I was immediately contacted by the insurance company. Apparently they track postings from this site. They apologized for the delays and worked out a settlement with me.

That really doesn't have any bearing on the matter. What does count is the language in your policy. If they claim that they only pay 40%, there should be something in the policy that says they pay only 40% for that type of claim. If that language is not found in the policy, they're trying to pull one over on you.
If they continue to be unresponsive, you should contact your state's department of insurance. Insurance companies tend to suddenly start paying attention when state regulators start asking questions.
