identification numbers, Oxford Health Plans, Inc. Health Providers identification numbers in its possession, upwards of 50 listings, claiming to be the Health Providers,

executing fraud. April 14, 2003 on going, Such Health Providers including:

St Lukes-Roosevelt Hospital Center, New York, NY, Amsterdam Avenue and West 114th Street

New York Presbyterian Hospital, New York, NY

Long Island College Hospital, Brooklyn, NY

Professional Claims Bureau, Inc., the third party, Debt Collectors, designates employees, Melissa Roth, June and Cassandra, Susan Frank, Milton Williams as Insurance employees for the purpose of:

Wrongful Disclosure of Individually Identifiable Health Information - "A person who knowingly and in violation of this part (1) uses or causes to be used a unique health

identifier; (2) obtains individually identifiable health information relating to an individual; or (3) discloses individually identifiable health information to another person

shall be punished as provided...

to be fined not more than $50,000, imprisoned not more than one year, or both;

if the offense is committed under false pretenses, be fined not more than $100,000, imprisoned not more than five years, or both;
