I think know what the OP is saying. A woman named Jessica Duck angry at the treatment of her father fraudulently claimed to to be the head of a disabled person's household. This was done to deny that person medical treatment. She is also angry that once this was reported this to the Pennsylvania Welfare Department nothing was done to Ms. Duck. The OP mentioned a police report she needs to follow this up with them and escalate as necessary.

I tried to make sense of that but really couldn't. In addition to spelling, a little use of punctuation and sentence structure would be very helpful.
Do people even read what they are submitting before they submit it?

hello jann liss at the welfare department at 914 penn ave #500 new that a clinic medical record tech has comitted fraud by submitting a form to the welfare office claiming to be another consumer as head of household and mrs liss never reported the lady whom was jessica duck, jessica duck forged a claim form signed a consumers name and welfare excepted it in april of 2011
