Mr. Dale submitted a cancel request on 12/30/2008 which is highlighted below. The request was processed on 1/01/2009 per his wishes. Once a cancel request is processed the account is closed which is why the login no longer worked. I checked our logs and do not show any phone messages from Mr. Dale or emails to either address outlined. We believe this to be resolved. I have personally emails Mr. Dale concerning this issue to see if we could be of further assistance in any way and await a reply back.
