The form is up to the state. Streamlining the form is not as easy as you may think. GEICO is basically 24/7 and they will walk you thru filling the form out correctly. No matter how irritating it is, it is your obliglation to provide the form filled out correctly within 30 days( which they are usually lax about ). Further more, I love when people say they only use a vehicle once or twice a month, guess what that can still be the day an accident occurs. Regardless it is not your fault there is nothing for the claims department to deny, all said I think you got off fairly well and learned a valuable lesson. Be thankful you did not find out after you had an accident that was your fault. Then you would really be up the creek, not out a few months of premium cost. You admitted the form was confusing, which it is, but yet now you are certain it was correct. Also how were you able to produce their copy? You have no idea what they have, unless you requested it officially? Even if you have a fax receipt copy, haven't you ever seen a fax come out totally distorted on the other side? Regardless you need to just be thankful you are alive and not atfault with no insurance.

I WAS able to produce the WAIVER they wanted, clearly showing I was WAIVING the UNINSURED MOTORIST option... Geico make's the forms confusing... because they WANT you to fill out the form wrong so they can SELL YOU MORE INSURANCE! In addition, they dont tell you what was filled out wrong, and your supposed to know what Geico wants when filling out their forms.
I was only riding the bike a few days out of the month, and I did not have the EXTRA MONEY that Geico wanted to add U.M. to my policy, but they added it anyways!
The law requires most of us to be insured while on the road, and while there IS a chance I could get hit by an uninsured driver.... I figured I be dead anyways. Bottom Line is I was compliant with the law and knew exactly what kind of insurance I wanted and could afford. I had the minimum insurance as required by law, I filled out the waiver form (and retained it) and Geico put me in a situation by not honoring my initial request. I was not at fault, and they would not have had to pay anyone had they not canceled my policy for "non-payment". They had no problem cashing my payment for 12 months coverage but did not give me a full year coverage.

Any motorcycle accident is horrible and I am sorry, however would you like to hear the irony of this? Say this accident occurred the day before the policy had canceled, and say the person who hit you did not have insurance, I bet that you would be very thankful your form was somehow "incorrect". Without that form and your signature you would not be crying "breach of contract" you would be crying pay me my money. If you don't want to pay for insurance that protects yourself while you are on a motorcycle no less, you are required to put it in writing that you understand the possible consequences. If unable to produce the acknowledgement that you understood that you would have NO coverage if you were seriously injured or killed by someone who did not have insurance; who do you think the jury will side with the "big mean corporation" or you the "misguided and never advised". If you still refuse to place it in writing take it up with the State Corp Comm - they are the ones that regulate how paperwork must be completed. Good luck.
