Hello:
On July 10th, 2006 at approximately 850am, I went to the Horton Occupational Health Clininc (16246 Prince Drive, South Holland IL 60473 708-333-4200). I requested a copy of my "long form physical" (previously completed Aug/2005). A woman who identified herself as Mrs. Horton stated (verbatim): "...Although the medical record is yours, it is owned by your employer and we cannot release it to you."

Advise the clinic that you are requesting a copy of your medical records under rights outlined by HIPAA (The Health Information and Portability Act)If they refuse, obtain the refusal to provide your medical record in writing from this clinic. Once you have the docmented refusal in hand contact the Federal DHS (Department of Human Services or their nearest office or an attorney of your choice) Good Luck HIPAA is a Federal law outlining a patient's access to their personal medical information. If they have them in their posession they must provide copies at your request or face possible fines and sanctions. You MUST get it in writing or it becomes a he said she said argument.
