revocation. The Department cannot represent you in civil matters to recover fees paid or seek remedies for injuries. You may wish to consult a private attorney regarding these matters.
The Department of Health investigates complaints and reports involving health care practitioners and enforces appropriate Florida Statutes.

HOW TO FILE A COMPLAINT/REPORT AGAINST A HEALTH CARE PRACTITIONER: To file a complaint/report, you must do so in a signed, written report. For your convenience you may use this form providing dates and details about your complaint.
Use a separate complaint form for each practitioner you wish to file a complaint against.
Be specific and include copies of pertinent medical records, correspondence, contracts, and any other documents that will help support your complaint.
Medical records are needed to process your complaint. Since a health care practitioner cannot disclose his or her patient names or records with authorization, the Authorization for Release of Patient Information form included on page 3 must be completed and signed. Signatures must be witnessed or notarized.
The Department will notify you in writing of the status of your complaint throughout the process. Please advise us of any address change.
If the allegations contained in your complaint/report are determined to be possible violations of applicable laws and rules, your complaint will be opened for investigation.
Please note that if your complaint is assigned for investigation, a copy of the complaint form will be provided to the health care practitioner pursuant to Florida law.
The Department may investigate an anonymous complaint if the complaint is in writing and is legally sufficient, if the alleged violation of law or rules is substantial, and if the department has reason to believe, after preliminary inquiry, that the violations alleged in the complaint are true.

They told me I had to be medication compliant at this place and take one of these psychiatic drugs with a side effect of "lower life expectancy" (shorter life) sooner death...
They said take the drugs or be thrown out, I was thrown out no refund as a lesson to the others.
Coercing something into another persons body is called rape by the way.
There was no way I was starting on that road of psychiatric drugging with the seroquel, risperdal or zyprexa that they give out at this place for that SCAM ! called "dual diagnosis".
These drugs originate from the chemical phenothiazene, which was used as synthetic dye in 1883. In 1934 Phenothiazene was used by the veterinary practice as an insecticide and extended for killing swine parasites; phenothiazene affects the brains Acetlycholinergic System and results in lethal increases of acetlycholine in the periphery and central nervous system of insect parsites !!
They give out these drugs to make people quite and compliant brain dead zombies !
They should call it "dual billing disorder", The major side effects include the client subject to toxic chemicals to induce a zombie like state, Major damage to there health and a life time of doctors appointments and pharmacy bills.
In short, the whole business of creating psychiatric categories of disease, formalizing them with consensus, and subsequently ascribing diagnostic codes to them, which in turn leads to their use for insurance billing, is nothing but an extended racket furnishing psychiatry a pseudo-scientific aura. !
F.Y.I, I understand that a DSM-IV diagnostic label has been assigned to me, based on my doctor's subjective judgment of my speech, manner, and behavior during our meeting, which lasted approximately FIVE ! minutes. I am aware that I will never be able to remove this diagnosis, or any other that will be added in the future, from my medical record.
I understand that although this quack doctor says that I am sick or that I have a treatable illness or disease, he or she is just using a figure of speech and cannot establish, with any test or procedure known to medical science that I in fact "have" the "illness" implied by the diagnostic label.
Indeed, I am aware that although medical opinion may now hold that a "chemical imbalance," a "brain abnormality," or some physical problem "underlies" or "produces" my "excessive beer drinking disorder", no objective information (through lab tests, scans, etc.) concerning the state of my body has been obtained in order to arrive at a DSM-IV diagnosis. If, by chance, such information has been obtained for that purpose, I understand that this information plays no role whatsoever in fulfilling any criteria for any DSM-IV diagnosis or diagnoses that I have been given by my physician except perhaps for diagnoses related to psychiatric drug-induced disorders such as tardive dyskinesia and lowered life expectancy !
I understand that the drugs that Oceanside Treatment Center coerces people to take cannot restore any of my physical or psychological functions "back to normal." Rather, the drug is expected to produce many new disorders in my brain and body !
Its called informed consent to treatment ! Every one gets a shrink label at this place !
