RCAP:  I received a phone call back from Catherine of RCAP 978-630-6707. I was finally getting somewhere, or so I thought. I told her that I have been trying to reach “anyone” since August of 2013. I then explained to her that I needed to apply for the RAFT / Homebase Program. She told me to leave a message with one of their call representatives “and they would get back to me within 24 hours.” This was the same number I had already left several messages with 978-630-6770. I left a message just for kicks.

I just called them again and was sent to Christina Wickford’s VM (for the Home base Program)

I was denied RCAP last year because I apparently signed a document that my landlord was supposed to sign. I have yet to see this document. They considered it fraud and I was denied any sort of assistance last year. I have been trying to get in contact with them and have never received a phone call back.

I am on the liver transplant list at UMass Medical Hospital. My Meld score is a 7, but I'm not able to go to work as I am disabled. (SSI and UMass Memorial Hospital have deemed that I am disabled due to having end stage liver disease). I lost most of my hair, medications make me ill and I'm tired all of the time.

I went to RCAP Solutions on September 26 @ 10:30 AM and applied for their RAFT Program. After sitting through their informational presentation, I met with Gheisha Diaz. She sat with me to complete most of the necessary paperwork to qualify for this program. Afterwards, she asked for a few more financial documents from me as well as my landlord. She mentioned that she would no longer be in her current position and that she was going to be training someone. I left with the necessary documents in hand.

I went back to RCAP (Gardner Office) on October 16 @ 10:30 AM with the rest of the paperwork that I had completed. I met with Gheisha and the woman replacing her, Tracy Parenteau. I was informed that all I needed to submit was a budget worksheet and there were still a few documents they needed from my landlord. I sent the budget worksheet via email on October 17 directly to Tracy via email. On that same day, I sent my new landlord a reminder letter that I needed the signed documents back a.s.a.p.

On the morning of October 22, I received a phone call from Carla of RCAP Solutions 978 630 6788. She asked why I was sending documents in when I had not even seen their contract. Apparently, I had called RCAP and spoke to her a few months prior to applying for RAFT. I explained to her that I HAD been in the office, twice. I signed and submitted most of the documents and had already met with Gheisha and Tracy. She informed me that I signed my landlord's name on one of the pages, where he was supposed to sign. She then went to yell at me, and said that she was going to report me for “fraud.” I had no idea of what document that I “forged” and asked her to send a copy of the document to me. I still have not seen this document. I TRIED to explain to her that in no way had I meant to commit "fraud" by signing my landlord's name. I had told my new landlord that we were applying for this prior to us moving in on October 1, which he was fine with. Why would I commit fraud? She was awful, just rude even. I was in tears

Two days after my conversation (if you consider yelling, conversing) with Carla, I received income documentation from the IRS and SSI/SSP. They were going to have to review my income. I’m nothing, I’m no one, why come after me?

I was denied assistance from RCAP for Fraud. I never received a copy of the document that I had fraudally signed. I kept calling. Carla no longer works there

My reason for contact you, besides the obvious, is that I applied for financial assistance with RCAP Solutions.  Apparently, I forged my landlord's name.  He eventually sent the original documents to RCAP in Gardner Mass and was fine with everything.

After meeting with RCAP three times all of which were fine and I was told that I was all set.  I tried to explain that the meds make me foggy and half of what I do, I cant even remember signing my name never mind forging someone else's name.

Now RCAP has all of my documents and refused me assistance because of this.

Is there any way that you could call 978 630 6788 and explain to her, the side effects of the various medications   They were originally going to help me with first,, last and security.  Obviously, that has been changed to second, last and security

Summary Process Agreement fo Judgement from Worcester Housing Court.

Our rental agreement that states we are renting the house on 5 Shady Lane in Templeton, starting on October 1 of 2012

Utility bill from Holden

I also told Gheisha that because of the 500 security deposit required when renting a house in Templeton, that we hadn't a clue how to pay this.

Income from SSI and SSPI

SNAP benefits

A letter from my Liver Transplant Surgeon @ UMass in Worcester

I was also informed that I scored a 7 on the one on one.

Once again I came into the Gardner Office on October 16 @ 10:30 am.  I sat down with Tracy & Gheisha and I was told that I needed to submit a budget worksheet and they are still waiting for the following from my new landlord.

Hi Gene,

As you know, we have requested rental assistance from (RCAP Solutions in Gardner) otherwise known as RAFT.  As you also know that any monetary assistance we'd be getting from this program would be put towards first, last and security. Or in this case, second, last and security.

Apparently, I signed one of the documents that you should have signed.  Honestly, with my huge list of meds, to prepare my body for the Liver Transplant, some things are still "cloudy."  I certainly did not sign this document with any type of fraudulent intentions.  In fact I believe that is one of the documents that I emailed you for your signature, which you sent off to me today.

A woman named Carla of RCAP solutions in Gardner, would like you to call her to verify/discuss this and/or any other issues that arise.  Carla can be reached at 978 630 6788.
