DJO Global billed me for a medical device for which I have already paid the provider, Verde VAlley Medical Center, Northern Arizona Healthcare. The date of service on the bill is incorrect by several weeks.
DJO Global claims, fraudulently, that I signed "electronically or on paper" a "DJO Global Patient Product Agreement Form."
I find it curious that the bill came from an address in Pennsylvania, and ordered payment to an address in Texas. I am also curious as to how this operation gained possession of my treatment information i.e. name of prescribing physician. I have not paid and will not pay DJO Global.
