I was contacted by Preston Kemp, who told me he was the HR manager for Durham Solutions.  He offered part time employment for mystery shopping services for Fortune 500 companies.  They couriered a package with a cheque in the amount of $1,150 in it.  He told me to take the cheque to my bank.  It appeared in my bank account to have cleared 5 business days later and so he gave me a simple WalMart mystery shop assignment.  Later that day I saw that the cheque had actually been reversed in my account.  After contacting him to inform him of this, I have not been able to get ahold of anyone at their office either by phone or email.

After internet research I see the forms they sent to me had been used in a US mystery shopper scam. 

I was only out the $106 I spent at the WalMart for the shop but I presume that if I had not waited for the cheque to clear the scam would have escalated to where some of the cheque money would have been returned or some other scam.
